Example Template Configurations

  • Subjective — Describe the patient’s symptoms, concerns, or changes since the last visit.
  • Objective — Document physical exam findings, vitals, or lab results.
  • Assessment — Summarize clinical judgment and diagnosis.
  • Plan — Outline treatment recommendations, medication changes, or follow-up plans.
  • Pre-Procedure Checklist — List all necessary pre-surgical checks.
  • Procedure Details — Document the step-by-step of the procedure performed.
  • Post-Procedure Plan — Outline post-operative care and follow-up instructions.

Tips for Building Effective Templates

  • Keep Sections Specific — Avoid overly broad sections; each should have a clear purpose.
  • Continuously Refine — Adjust templates as clinical requirements evolve.
Adding instructions and an example for each section (e.g. Section: “History of Present Illness,” Instructions: “Describe the chief complaint, onset, duration, and pertinent positive/negative symptoms,” Example: a short sample note) helps ensure consistent, accurate data entry.
